Urban Health Plan (UHP), one of New York State’s largest federally qualified health centers, serves communities in the Bronx, Corona (Queens), and Central Harlem. With a holistic approach to care, UHP offers a wide range of services across 12 care centers, 12 school-based sites, behavioral health clinics, specialty care, home visits, and community food programs.
Since joining WholeYouNYC in February, UHP has made a strong impact by screening, and navigating Medicaid members to essential services in the community. As a leading participant in WholeYouNYC Care Connection Collaborative for Healthcare Partners, UHP consistently implements data-driven quality improvement strategies and serves as a resource for other healthcare partners in the network.
