WholeYouNYC Announces Major Expansion with New Social Care Network Initiative

We are excited to share that Public Health Solutions’ WholeYouNYC has been awarded the Social Care Network (SCN) infrastructure grant, serving Brooklyn, Queens, and Manhattan. This remarkable achievement acknowledges the hard work and dedication of our diverse community partners, and we are excited to embark on this journey to improve health equity across the city.

Serving over three million Medicaid members across PHS’ three regions, WholeYouNYC’s Social Care Network will screen Medicaid members for health-related social needs and connect them to community resources and Medicaid-funded services, such as care management, food and nutrition services, housing supports, and transportation assistance.

WholeYouNYC partners with over 300 diverse community-based organizations (CBOs) across NYC. This network of CBOs play a vital role in connecting Medicaid members to tailored resources. The infrastructure grant supports training, technical assistance, and capacity-building for CBOs to help them deliver high-quality services to eligible members.

Through this new initiative, New Yorkers will be connected to a more equitable and integrated social and health care system. Together, we are creating a future where everyone in New York City can thrive.
